NEW CUSTOMERS

All new customers please be aware you are required to print the following Request For Service PDF. & provide payment method before any work will be undertaken. This form will need to be either faxed to (02) 9609 1411 or emailed to service@hdscientific.com.au

Please be aware that the Decontamination Declaration is required to be signed before any work can be carried out on any of the following equipment:

Centrifuges
Cryostats
Microtome's
CO2 Incubators
Fume Absorbers
IVF Chambers
Frontell Grossing Stations
Any other type of equipment used in the analysis of blood, human or veterinary tissue

EXISTING CUSTOMERS
